You get the next available CSR regardless of where they are. DTV's support number is 1 800 531 5000 that they had before the merger.

There is no way to request location of agent you speak with. Just like you cannot request gender, cultural background etc. It is the next agent regardless.
The 800-288-2020 is AT&T's number which has more traditional business hours/days.
DirecTV still has their original number 800-531-5000. This is preferred as there was not a good track record of transferring calls between the two sides.
Online chat or a Direct Message (DM) from the official forum help team are not guaranteed to be in-country either.
Alternatively, sometimes the forum community can help depending on the situation. Sometimes we can help find an answer or at least give guidance. Feel free to post details about what is going on if you want others to try and help.

Are saying if one gets a rep with a heavy accent that the caller can't understand, they are stuck or have to hang up and try again and hope they get one that can be understood?

That would be true even if support was United States only. There is not one perfect voice for everyone. There are different accents and dialects across the country.
So yes, if you cannot understand an agent then you hang up and callback. Though likely is the reason for the request, the OP did not say that is the reason. There are other worse and common reasons for that request.
