On Demand charges

Has anyone experienced phantom bill charges for On Demand movies you never ordered? My Mom is 93 and there is no way she would be able to figure out how to order a movie. Not to mention, there are several steps (i.e. previews) involved before you can actually place an order. She lives alone and even if she did figure out how to order movies, the types of movies supposedly ordered or not something she would watch.

Has she had a set top box or DVR changed recently?  I can't answer about U-Verse, but I had this happen to me with Comcast when I had a DVR removed (so I could replace it with a TiVo); the tech took the box, but didn't leave any receipt as another tech had to come back the next day, and after I called support to have the monthly charge for the box stop appearing on my bill, I started getting phantom movie charges at times when I wasn't even home.
