In our case we did not have to pay an installation fee. We had TV services, cancelled TV and then added it back later under a promo to get a gift card for coming back. I don't recall if we added a new promo when we added TV. Call customer service or go online and change your plan to see what you get charged.
There are several levels of U-Verse internet you could have. Some of them allowed for Customer Self Install, others don't. If you had a self-install, then you will need a U-Verse "professional install" to qualify for TV service, and some of the higher tier internet services.
Also, if you haven't had TV service before, then your RG will not be one which supports TV, so that requires a change, and proper configuration. This is best when performed by someone who knows what they are doing.
If you watch the specials, you can often get a "free installation" deal from AT&T (but this is not guaranteed).

Even if you have to pay for the install, any promotions or other discounts should be enough to offset the cost of the install.
