Account Number
We signed up with Direct some months ago, I did not fill out this form. Our payments are on Auto. I would like our Directv account number, James(Jim) and Donna (Edited as per community guidelines). The Auto pay is with Chase United Awards last four digits of card are (Edited as per community guidelines). I hope this is enough information for you to find it.
