AT&T discount card $200 expired and not able to reactivate again!!!

I just found out that my discount card of $200 that was offered during point of sale is now expired. First of all, I don't even recall the card being received in the mail. And I can see on my AT&T portal that the reward card (AT&T U-verse TV U300 all included 2-Year Price Guarantee Offer with Bundle includes Reward Card) expires on 09/27/2018. When I called the reward services to see how to redeem this card, customer rep told me that the reward card was mailed out in 2016 and since it was not activated, it has been already expired on Jan 2017 and he cannot resend it again ! How's that possible? I am still using services and under promotion period. Why was there no communication made through the email? 

Also, the AT&T store rep this week told me that my promotion period is expiring this week and not next year and tried to sell me "direct tv".

thanks for the response, however that's not the store rep told me. She told me that the price will go up in 1 week  ( $50.15 is the bundle price discount offered a year ago). The rep over the phone today told me price is guaranteed for 2 yrs. 

What are your thoughts for $200 discount card? Why is that already expired and cannot be mailed again?
