How do I report a wreckless AT&T Driver?

I almost got ran off the road today by an AT&T service vehicle. I was northbound on US-101 in the Mountain View, California area, and this driver changed 3 lanes at once to try to get to an exit. In the process, he crossed so close to the front of my car that I had to swerve onto the shoulder to avoid him, and we were going nearly 70 MPH. I didn't get a look at the driver; I was so shocked.

The vehicle was a white pickup truck with AT&T logos all over it, with license plate: 33454M1

Same thing just happened to me. It was on 280 North before the Skyline exit. The driver pushed me into the shoulder as he was merging from the second to the right lane. He was trying to make the Skyline exit. If I had not slammed on my brakes I would have hit the wall next to the shoulder. Is there a number to report the driver? His LP was 6GP9010.
