Max streams out of home with Directv Now grandfathered plan -2 streams but pay for a 3rd stream
I have had directv now go big from day 1, and am grandfathered in. How many streaming TV devices can I access out of my home if my plan includes 2 streams, but by paying an extra $5 I get another stream supposedly? I read the general rule that directv gives you 20 streams inside your home and 2 streams to tv devices outside the home. What does paying for a 3rd stream get me? It appears not a 3rd outside the home because I tested it and when I tried to stream a 3rd tv, I get a message saying too many streams when I try 3.

I’m using an Apple TV, Roku tv and Samsung tv.

They've never really said what rules apply to those of us that are grandfathered (other than the various things we can't get). But if the rules for "away from home" that apply to those that get the 20 streams apply to us also, then with 3 streams, you can use all 3 at home, and you can use all 3 away from home, but away from home only 2 of the 3 can be "TV streamers", like AppleTV, Roku, or Samsung TV. The other would have to be a mobile device (phone or tablet). (All 3 could be that.)

When it says 2 streams to TVs outside the home, does that mean only 2 can ever be logged in? I have 2 homes, and with 3 TVs in each. When I try to login the 3rd tv at the second home, even though no other tv has been on for hours, I get an error. Does the 2 TVs outside the home mean even if turned off, only 2 TVs can be used outside the home?

It's not just enough to turn off the TVs. You would need to log off your DirecTV Stream account on that streaming device. What streaming devices are you using at the second home?
