Direct TV $100 Visa Card Never Arrived

We were told when we signed up for Direct TV that we would receive a Visa card $100 offer in the mail three weeks from installation - that was well over a month ago. It never arrived. And I can't seem to get any help on Live Chat or by phone.

This is what is in the sales confirmation email:

AT&T Visa Reward Card Reminder:

Watch your email and mailbox for reward redemption instructions. They will arrive within four weeks of installation of all qualifying services.

Nothing after that!
