The site I was sent to didnt have a permission form for HOA. It had a lot of outdated posts concerning this issue. However I didnt see a form or resolution.

DirecTV does not provide an HOA "permission form".
It's against FCC regulations for an HOA to restrict your right to install a satellite dish in any area under your "exclusive use", such as a deck or patio. Do a Google Search for "FCC OTARD" to see your rights.
