major network feeds

when we changed to Direct TV we were told we would be able to access all major networks no matter where we were on a 4 months cross country trip we were planning. This was VERY important to us so my husband could watch NASCAR. We now are in Ohio (live in Arizona), and cannot access major networks and was told we need to complete a form for the fcc.  Tech support person was supposed to e-mail last night but didn't. How do I download this form to get this handled?  My husband wants to watch NASCAR and can't.
