Returning Equipment after canceling DTV?

I canceled DTV 2 weeks ago. I confirmed several times during the call that I would receive an email directing me how and where to return the equipment. I still have not received an email from DTV or AT&T. Please let me know how and where to return the DTV equipment ASAP.

When did you cancel your service? Please note that equipment is required to be returned within 21 days from the date the account was cancelled.
