What on Earth! Ironic title for broken DVR recording

So the DVR is basically useless anymore.  Above you will see that I have a program set to record the show "What on Earth" - First Run only.  I get 5 shows recorded in the same day that are old.  And then, when I watch them them they are all actually the show "Through the Wormhole", and old as well...

What going on?  This happens all of the time - the DVR fills up with garbage.  I also have the problem where one show will be 4 hours in length but is actually 4 shows with the same name in succession.  It blows because only the first show is new.  It seemed to happen all of the time with Ancient Aliens.

Is this happening to others?
