Promotions
I have noticed every month my Direct TV bill is different. I also noticed a movies package on my bill. I have had the same packages since early 2000's, Direct TV, before AT&T. Long time customer. Calling they said I added a promotional package in February. No, I called in February to get an amount, because i didn't get a bill. Talking to the representative, I also found out they billed me $5.00 for a reprint. I didn't ask for a reprint, just a $ amount. I looked back at previous statements and saw where I was charged a prorated amount for a package ending, in May. I did not order an additional package. I asked for them to credit me the 23.66 the total that I have paid for this package. They said absolutely not. They would cancel the subscription (that I did not order) and it would be prorated. I am currently looking into other options, as this was mainly for my mother. I'm sure with a bill over 250 every month, I can find something. They definitely aren't the company they use to be.

Outside of the annual price adjustment (new rates based on increasing costs from networks), the bill should be consistent.
Calling and asking for an amount and they selected a reprint? Unless you got a new agent who chose the wrong option that sounds like they misunderstood what you wanted. With the change in DirecTV's ownership I would be very simple and clear on wording anytime you call (though I would suggest in general as call center agents are overworked by high call volume and required to keep their Average Call/Handle Time low).
Any add-on packs like that do not start/stop automatically. They have to be added (call-in, website, app, or from the DirecTV box itself) and removed (by call-in). They should have been able to see what method added it so you could take precautions (especially if you have a household member adding things without your permission, even if by accident such as with the remote).
I would suggest anytime you contact a company (not just DirecTV) review the next bill that produces after that. That way you can verify any changes made were as expected and no unexpected ones. If you did make changes, I would look at the next bill as well since the 1st would show proration for anything mid-cycle and the 2nd regular bill going forward. Beyond that, at least look at the amount each month. That way when you notice a change you can review it then and there instead of months later (or years for some inattentive people).
Yes we want our bills correct, but there is an amount of responsibility on our side to do our due diligence. Most companies have an expectation of being notified of any bill disputes within a reasonable time, which for DirecTV is 60 days.
With the couple charges hopefully that is the end of it and not the norm of issues. I would still review bills on a decent regular basis. Since you mention having the same package for about 20 years, good chance a current package would be a better fit. Some channels moved to lower costing packages they used to be in, and other channels which required a higher package no longer exist. So I would review the channels you watch and see what current package has them to see if you could move to a lower costing package.
