What is the receiver model? Does 721 occur on all channels or only some? If it happens on all, you’ll need to go online and sign into your account at DIRECTV.com
From there, go to Manage my equipment and refresh the receiver that has error 721. If the error doesn’t go away, try a red button reset on the side by the access card for the HR54 series or the front where it says ‘PRESS’. Leave the receiver overnight and try another refresh the next day. If error 721 persists, your receiver probably needs a new access card as the old one isn’t accepting authorizations. Call DIRECTV (number in my greeting below) and ask for a new access card for the receiver.
Error 721 means it thinks that channel is not subscribed. If it is a simple authorization error, you can normally refresh it yourself: www.directv.com/refresh

What model box?
What channel? (or is it many or all?)
All boxes affected?
Exact name of programming package?
Anything changed before this happened?
