You visit a UPS Store with your equipment and your U-verse Account Number. The UPS Store guy will confirm what equipment you're supposed to return, give you a receipt for it, then pack and ship it back to AT&T. Don't forget your power cords. Hold on to that receipt for a few months past any chance AT&T has lost the fact that they've received your equipment.
