sending back equipment

we recently moved and had our equipment reinstalled at our new home. the service tech gave us some ol equipment and a box and said we needed to send it back to ATT/uverse. He did ot provide who we should use to send it back or a label to do so.  How do i send this equipment back?

Equipment is returned by taking it, and your U-verse Account number to the nearest UPS Store.

RE: Returning U Verse equip package, TV & Internet.

 

I am in the hospital, cannot get to a UPS store. My recovery and rehab time will be very lengthy. The equipment was initially installed in my apt. by a tech, I was originally solicited at my door of the apt. by two people, I'm no longer at my apt. nor will I ever be again, why can't a Tech go to my apt. to pick up every piece of equipment listed to return?
