I use captions all the time. I have DirecTV satellite, and I find I have to do an actual restart of my STB every month or so to get captions to work properly. The issue that I have is only 1 line of 2 would come in, or they would be missing altogether for several seconds. I find the restart fixes the problem.

If it's just two channels then it's likely an issue with their broadcast.
What DVR/Receiver model?
Have you tried resetting the DVR/Receiver?
