ESPN Unlimited no longer working

I received ESPN unlimited a few months ago, activated it and it worked fine up until about a month ago and now I can't access it. It keeps telling me to link my provider, and the link doesn't show anymore in my tv package or account information, probably since it was already activated. How do I fix this?
