form for buy out
I need to get a form from direct tv so my new cable company will buy out for early termination.
Early termination that was never agreed to it, was automatically renewed by just paying the bill.
I don't believe this was legal. Direct tv changed over to an HD system and we were REQUIRED to change
over and were told there would not be any charges, but there were. Been a customer since 2014 and now they are charging us $427.23.

It appears you where upgraded instead of getting the free SD to HD swap. The email DTV sent before the install would have stated which one you where getting.
AFAIK DTV doesn't have any form for that you cancel the service then DTV sends you the bill with the ECF/ETF and you submit that to your new provider.

There is no "form" to provide to another company. Why would any company provide a form in favor of swapping to another company? The Early Cancellation Fee (ECF) shows itemized on your bill. If your new company reimburses you for the ECF, then the itemized bill should be all you need.
You are NOT automatically renewed just by paying the bill. That is not how it has ever worked. Sounds like what actually happened was a different order placed then what was expected.
DirecTV has been shutting down the outdated MPEG-2/SD-only feeds. For customers with that very old equipment they have a MPEG swap program which is free equipment, new monthly charges are credited, and no new service agreement. Alternatively you can do an optional upgrade which makes the new charges valid and comes with a 24 month service agreement, which is usually if someone wants different equipment then what the swap would provide. So most likely the order was an upgrade, not the complimentary MPEG swap. The order confirmation would have clearly shown these details, which is why it is very important to read through before the new equipment is installed.
Yes fault on agent for wrong order placed (by accident or on purpose). But order confirmation shows all charges and service agreement, and signing off with the installer's handheld has in the print the 24 month agreement as well for a last chance notice, so in the end you accepted everything that was on the order.
