Please help

My husband is getting third degree of identity theft on him because we have gotten a gift card in someone else name that wasn't my name on the account when we was setting up the account they got my name wrong I need help because he needs it for court soon.. I have already call people about it long time ago they told us to get online to fill out something for it I don't know we're to go so if anyone can help me that would be great
