Would like for this issue to be resolved and service cancelled permanently. Would also like to set up time to meet with someone to pick up equipment

DTV doesn't pick up anything. If the receivers are returnable you take them to a UPS/FedEx store to scan them in and kept the receipt as proof you returned them. You have to call DTV to cancel the account 1-800-531-5000 but DTV no longer prorates the last bill so you have payed for and receive service till the end of the billing cycle. DTV will only suspend the account for six months per year and it will automatically resume.

There is no "until further notice". Most likely what happened is agent suspended (not canceled) your account. That is for a max of 6 months within a 12 month period, at the end of which it automatically resumes as you cannot hold service indefinitely.
To cancel service, you call DirecTV. Be aware that cancellation goes through at end of service cycle. There is no proration/adjustment for unused days.
Nobody ever picks up equipment. If you have any returnable receiver/client boxes, you take them to a participating FedEx/The UPS Store with your account number for the free return. Make sure to keep receipt showing they took possession. All other equipment (dish, cabling, etc.) is part of your property once installed.
