It depends on your demand. If you want a wired receiver, then just call DIRECTV up to order it and have a technician come and install an additional coaxial cable to that room. Depending on stock, the client would either be a C41, C51, or C61. The only thing you would need in this case is a coaxial cable in that room and a Genie mini receiver.
However, if you want a wireless receiver, you will need to call DIRECTV and ask specifically for wireless. Wireless does not require a coaxial cable to be run to that extra room. However, it does need a Wireless Video Bridge (WVB) to be connected to the HR44 so that it can 'bridge' the connection and create it's own network for the wireless Genie receiver. Depending on stock, this could be the C41W receiver or the C61W receiver.
Although DTV may not currently offer it due to stock, you can also add-on an additional HD DVR receiver (HR24) for extra DVR storage. These boxes are standalone and do not require the Genie HR44 receiver to work. Additionally, there are also standalone HD receivers such as the H24 and H25, which don't require the Genie to work either.
Some disclaimers are that Genie mini receivers (wired or wireless) take away a tuner from the main Genie receiver. If this is problematic, your best route would probably be a standalone HD or HDDVR receiver. Obviously, an extra receiver on the account will add an additional $7 per month to your bill because of the TV access fee.
