No, the Movies Extra Pack is not including MGM+. As stated before, if you currently subscribe to EPIX, the name will change to MGM+ and you will continue to be billed that way. No change in price will occur.
The Movies Extra Pack used to be called the HD Extra Pack. Set of channels (564 thru 573). Was at the early days of HD, so was a set of speciality channels. Couple channels have changed over the years and little known is that Hallmark Movies & Mysteries (HM&M) is now in Select, Entertainment, Preferred Xtra, and Premier.

What HD package? Every package DIRECTV offers has HD in it (even the lowest package Family) so I’m confused.
If you want MGM+, you get the EPIX add-on.

Since DTV is shutting down the SD channels one would assume so. MGM+ is a subscription service with its own fee.

Unfortunately MGM+ and Epix is not part of it.
