Why does my directv app not work out of the US?

We keep getting calls at the water department for the manhole around 359 Queen ST Southington CT.  Water has been draining out of it for weeks.

I just got my Denon receiver connected to my tv and directv box.  On some of the movie channels the audio and video are out of sync.  I currently have the directv box connected to the TV with a HDMI cable and then a digital optical cable from the directv box to the denon reciever.

Why is the movie channel not apart of the premium add on package paramount+ with showtime on directv stream?
